For practitioners updating or adding their report delivery information for laboratories within the Fraser Health Authority, an external registration process is still in development. 

For patients which present at a Fraser Health Authority laboratory with a Standard Outpatient Laboratory Requisition for Pharmacists, the laboratory will employ a interim process to register pharmacists in their system. 

Note that the requisition must have complete and accurate practitioner information.

Please complete the Physician Activation e-Form and submit it directly through the link located on the bottom of the form. 

Please complete the Healthcare Provider Information Change form (PDF) and send it via fax to 1-877-412-4440 or local 604-412-4445; or you may print and forward it via your local LifeLabs courier.

Please ensure you are first authenticated in NH Cerner to receive any results or reports. 

Then, complete the  CDX Activation Form for the Clinic to which they are receiving results, as well as the Information Sharing Agreement (ISA)

Submit each document directly through the links located on the bottom of each form respectively.
