Updating Physician Information

To update physician information, you will need to complete a laboratory provider change request.

Please fill in the provider change request form (PDF) and email to: labphysupdates@phsa.ca.

This form can be used to:

  • add a new MSP provider
  • add an additional location for an MSP provider
  • modify/replace an existing location for an MSP provider
  • remove a location for an MSP provider
  • add a new non-MSP provider
  • add an additional location for a non-MSP provider
  • modify/replace an existing location for a non-MSP provider
  • remove a location for a non-MSP provider
  • add a new non-MSP clinic/facility/location
  • modify/replace an existing non-MSP clinic/facility/location
  • remove an existing non-MSP clinic/facility/location
