To update physician information, you will need to complete a laboratory provider change request.
The process to update your contact information varies by health authority or organization.
Please go to your organization' s website to access your provider directory. In addition, we urge you to inform the College of Physicians and Surgeons of BC of any changes.
For providers updating or adding their report delivery information for Vancouver Coastal Health, Providence Health Care, BC Cancer, BC Children's Hospital, BC Women's Hospital , and BCCDC laboratories, please fill in the
provider change request form (PDF) and email to:
