Make Your Conflict of Interest Declaration

When to declare

You are required to submit a declaration form every year. The date that the form is approved becomes your anniversary date; unless a conflict comes up in the meantime, you will need to submit again at the same time next year. We will remind you before this date comes up. 

You may need to submit a new declaration form before your anniversary date comes up again. This is required if:

  • Your role, responsibility, or research changes and it results in conflict
  • Your financial or external interest changes and it results in conflict
In this case, you should update your form immediately. Approval of your amended form will result in a new anniversary date and you will not be required to update your form for another year.

You must be connected to a health authority/research institute network to access the COI declaration.

How to declare

If you have determined that you have no conflicts as outlined by PHSA's Research Conflict of Interest Policy AB 207, you simply need to complete and submit your declaration form. Please refer to Research Conflict of Interest Protocol, a step by step guide to assist with COI declaration.

In the event of a conflict, PHSA requires that you develop a plan to appropriately manage the conflict. You will be asked to outline your plan in your declaration form.

To make your COI declaration, you must log in to the PHSA COI Declaration website to access your form. You will be able to submit, renew, change and review your declaration here.
