Access

Methods of Access

Access to the eForms platform is controlled. There is more than one way of authenticating to - and accessing - eForms.

Launcher v. EMR

Many users will simply open their Internet Browser and go to a site which launches the eForm. This is referred to as the eForms Launcher

EMR users will view and submit the forms via their EMRs. All access within an EMR is controlled by the vendor. For more information, see the EMR section of this site.

Launcher Authentication

Launcher Users can be further broken down by the kind of authentication they use.

  • Health Authority
  • Non-Health Authority
  • Provider Identity Portal

Requirements
  1. An active health authority email address and password.
  2. Access to the health authority network or VPN.
  3. A completed eForms Enrolment Spreadsheet with a list of all users who require access.
How to Fill Out the eForms Enrolment Spreadsheet 

Fill out the eForms Enrolment Spreadsheet as follows: 


First and Last Name: A preferred name can be used if you have access to a health authority network or VPN.
  • If you require access at a location without access to your health authority network or VPN, access will be via the BC Services Card app, and the first and last names provided must match the names associated with your BCSC profile
Department: Your department or organization name.

Email Address:
 Your health authority email address, if you have access to your health authority network or VPN.
  • If you require access at a location without access to your health authority network or VPN, access will be via the BC Services Card app, and the email address provided must match the email address associated with your BCSC profile.
Form: Indicate the form(s) you require.

Phone: A general organization or department number. 
  • One phone number can be shared amongst multiple users on the seame team, but it must be an active number and appropriate to that group, such as a main workplace line. 
BC Services Card App

If you requires access away from your health authority network or VPN, you will use the BC Services Card app.


Visit the BC Services Card Enrolment page to begin the process of setting up the BC Services Card app.


It is recommended that you start the process of enrolling for the BCSC App before you submit the eForms Enrolment Template Spreadsheet.


Non-Health Authority Users will authenticate to the eForms platform via the BC Services Card (BCSC) app.

Requirements
  1. The BC Services Card app installed on your mobile device and correctly set up.
  2. A completed eForms Enrolment Spreadsheet with a list of users who require access.
How to Fill Out the eForms Enrolment Spreadsheet 

Fill out the eForms Enrolment Spreadsheet as follows: 


First and Last Name: First and last names as they appear on your BCSC profile.
  • Note: The first and last names must be entered exactly as they appear on your BCSC profile or authentication will fail and you will be unable to access the eForms application. (E.g., Entering 'Chris' when the name appears as 'Christopher' on the BCSC profile will cause authentication to fail).
Department: Your department or organization name.

Email Address: The same email address associated with your BCSC profile. 
  • Note: If an email address is provided that differs from the one associated with your BCSC profile, authentication will fail and you will be unable to access the eForms application.

Form: Indicate the form(s) you require.

  • The same phone number can be used for multiple users on the seame team, but it must be an active number and appropriate to that group, such as a main workplace line.

The Provider Identity Portal (PIDP) is a BC Government - Ministry of Health initiative to streamline authentication for BC Providers, for various electronic systems.


Like other non-health authority users, you will be authenticating with the BC Services Card (BCSC) app. However, eForms does not actively onboard your ID. Rather, you register with the PIDP site and acess to eForms is granted automatically.

Special Authority eForms

Use

Most eForms take advantage of the built in search features listed lower in this page. They reference the following data repositories:

  • EMPI (Enterprise Master Patient Index)
    This is the source of truth for patient demographics, for the province.
  • PLR (Provider Location Registry)
    This provincial repository for practitioner information receives data from the respective colleges, such as the College of Physicians and Surgeons.

Client Search

Client Search allows the form filler to search EMPI for the patient referenced in the eForm. Patients can be looked up by Personal Health Number (PHN) or a combination of first, last name and date of birth. 


Once a patient is found and selected, the form pre-populates the patient fields. These can be modified, if needed. Simply type over or delete and re-type to make corrections. This will not alter the record in EMPI - but may be useful for an individual submission.


If a patient wants to update their identity or address information, they must be redirected to Insurance Corporation of British Columbia (ICBC) or Health Insurance of British Columbia (HIBC) with documentation of proof.


The information displayed in the Patient search before the form is loaded and prepopulated cannot be modified by the eForms Team. As noted above, eForms DO NOT maintain this database - it is managed by other provincial authorities.


Provider Search

The Provider Search feature enables users to look up healthcare providers/practitioners who are listed in PLR. 


As with the patient look up, once a practitioner is found and selected, the form pre-populates the provider fields. These can be modified, if needed. Simply type over or delete and re-type to make corrections. This will not alter the record in PLR- but may be useful for an individual submission.


If a provider needs to update their information, they must contact the college that governs their practice. Once the data is corrected in PLR, it will cascade to the eForms system and display correctly in Provider Search.


The information that is displayed this search cannot be modified by the eForms Team as we DO NOT maintain this database.
