Update January 4, 2022: Due to changes in B.C.’s COVID-19 testing guidelines, the PC-ICCN will be reviewing eligibility requirements for the Post-COVID Clinics to ensure continued equitable access to care for patients who continue to experience symptoms longer than 3 months.
To refer a patient to a
post-COVID-19 recovery clinic, please fill out the
referral form and fax it to central triage: 604-806-8809.

Please note that referrals will only be accepted for:

Confirmed COVID-19 positive (NAAT or serology) or
official epi-linked cases
- Individuals who were symptomatic between January and May 2020, and did
not have access to a COVID-19 test
If your patient does not have a positive COVID-19 swab, they will require a positive serology test to be accepted into the Post-COVID-19 Recovery Clinic. However, referrals will be accepted for patients who were ill between January and May 2020 – even without a positive test – because testing was less accessible during the early stages of the pandemic.
