How to Request Access to CareConnect? Please select your work setting below:
I work in a Health Authority Setting

For example:

  • Acute and Ambulatory
  • Health Authority Community Clinics
  • Health Authority Employees
  • Hospital Locums

Please note that if you have access to CareConnect in one setting, you still need to request access for the other setting.

For example: If you have access through a Health Authority role, you will not automatically have access in a community clinic (and vice versa), and will have to apply again through the “Community Based Health Provider” pathway.
