Accreditation
Accreditation is a mechanism to ensure high quality laboratory services to patients and physicians by defining mandatory and best practices in laboratory medicine. Accreditation organizations send representatives to our laboratories to assess compliance to the requirements.
PHSA Laboratories is proud to have its laboratories accredited by the
BC Diagnostic Accreditation program (DAP) for Laboratory Services. All laboratories have had on-site accreditation visits in 2009.

The PHSA Laboratories Quality Management System
PHSA Laboratories uses the CSLI Quality System Model. This model is compliant with ISO 15189 and ISO 9001.
All PHSA Laboratories have Quality Manuals that include sections for all quality system elements as shown in the table below. The 12 QSEs shown below, the building blocks of quality, drawing substantially from CLSI quality systems materials.

Many processes are defined, well on their way in process improvements and are audited periodically. A risk assessment process has been added to our Assessment section of the Quality Manual in 2009 to assess current practices when required. There is a PHSA Laboratory Safety and Quality of Care Committee (SQMCC) that meets regularly and reports to the PHSA Board.
PHSA Laboratories utilize Lab QMS™ software for document control, equipment and audit functions and the Patient Safety and Learning System (PSLS) for reporting, handling and management of occurrences.
